Federal Verification Forms- Complete and submit one of these forms only if you are notified to do so by the Office of Financial Aid, indicating that your FAFSA was selected for federal verification. The forms are the same for all populations or degree programs.
Parental Information Waiver- Use this form if you are a medical student who wishes to request a waiver for parental information.
Financial Aid Request Memo – Use this form to request increases or decreases in your current aid award.
Emergency Advance Form – Advances up to $1,500 of aid from the next semester up to 30 days prior to the normal disbursement date.
Emergency Fund Application - Assists Medical students who are faced with catastrophic or emergency events that result in unexpected hardships which require short-term immediate financial assistance. Students applying for this fund also need to complete a W-9.
Tufts MD Residency Travel Fund Application (not yet available) - This fund was created by an anonymous donor to assist financially needy students by defraying the transportation and hotel costs associated with the residency application process.
Third Party Release Authorization – Fill out this form to give the Office of Financial Aid permission to release or discuss your information with a third party.
