Become a Clinical Instructor
Interested in offering a clinical education experience for Tufts DPT students? If your clinical education site is new to Tufts DPT, connect with Devann L. Leech, director of clinical site development. If your site is already a contracted clinical partner, please email the DCEs of Tufts DPT-Boston, DPT-Phoenix, and/ or DPT-Seattle listed in the sidebar. We look forward to your partnership.
CIs are encouraged, but not required, to take the APTA’s Credentialed Clinical Instructor Program (CCIP) Level 1 course, which is now offered both virtually and in-person through the APTA. If you are interested in having a CI course for your specific facility or group, contact tara.dickson@tufts.edu.
APTA’s Level 2 CCIP course details how to develop a clinical curriculum for your facility, and CIs who have taken the Level 1 are encouraged to continue their education in this way. See below for additional clinical teaching resources.

- CIET Training Video
- Use this link to watch a short training video of how to rate clinical performance on the CIET.
- CIET Paper Version
- This is a paper copy of the CIET for your reference. It is strongly preferred to have the student and CI complete the electronic version of the CIET through Exxat to ensure consistent records. However, feel free to use this as a backup as needed.
- CIET Benchmarks for Each Clinical Education Experience
- These are the program expectations for CIET achievement for the Tufts DPT students during each PT Practice course. These can also be found in the PT Practice course syllabi and the Clinical Education Handbook.
- Feasibility of the use of the CIET in Full-Time Clinical Education Experiences
- This 2022 JOPTE article found the CIET to be easy to use, providing an “efficient, valid, and reliable assessment” for DPT students.

- Blank CSIF
- This is a slightly shorter version of the CSIF (Clinical Site Information Form) for SCCE use
- APTA Student Evaluation of the Clinical Education Experience
- This is the paper version of an APTA document that students complete on Exxat that has students evaluate CIs and the overall clinical education experience. It is here to serve as a reference.
- DPT Student Goal Writing Worksheet
- The purpose of this worksheet is to encourage students to share their goals for their clinical education experience and overall professional development, creating a space for discussion of these goals with their CI and faculty coach. This is best used at the beginning of a student’s clinical education experience and should guide students with weekly goal writing and CI check-ins.
- Weekly Planning Form
- It is highly recommended that students use this form to proactively guide and plan weekly learning goals during the clinical experience
- Clinical Education Planning Sheet
- This form can be used to identify and communicate specific areas in which students are struggling during clinical education. It is encouraged that the CI, student, and DCE work together to identify areas of concern. In many cases, students can be empowered to identify specific objectives and learning experiences to achieve their clinical education goals and requirements.
- Anecdotal Record
- This form can be used to initially document any concerning behaviors, safety incidents, etc. that arise during the clinical experience. Either the CI or the evaluator can be the initial person to complete the form. The DCE MUST be contacted for any safety issues or other areas of concern that occur during clinical education.
- Critical Incident Report
- Like the anecdotal record, this form can be used to document any critical incidents (including safety or professional concerns) that arise during clinical education. This form is more useful for incidents that have occurred more than once. The DCE MUST be contacted for any critical incidents that occur.
- Guidelines and Self-Assessments for Clinical Education
- This 2004 document by the APTA includes self-assessments for CIs and SCCEs for the purposes of reflection, development, and growth of clinical teaching and clinical education experiences.
